PRICING

Sessions are charged at time of service for an individual session. 

A significant discount is offered should you choose to bill via a monthly subscription process.

Monthly Subscription

Physical Therapy is often provided over a course of time. You may opt to participate in our monthly subscription program which offers a discount on services. 

Payment for services is collected at the beginning of the month for the subsequent month's services. Discounted rates are as follows:
- 60 minute physical therapy in person session: $160
- 45 minute physical therapy in person session: $125
- 60 minute Telehealth physical therapy: $125
- 45 minute Telehealth physical therapy: $100

INITIAL EVALUATION

Duration: 55 minutes

$225

Evaluation and diagnosis via an in-depth interview and clinical assessment which looks specifically at your movement, strength, range of motion, and uses specialized tests in order to determine an orthopedic diagnosis and treatment plan.

FOLLOW UP SESSION

Duration: 55 minutes $185

45 minutes: $150

Treatments include:

Manual therapy: 

techniques utilized to gain range of motion or decrease pain.

Therapeutic Exercise: specific exercise that targets muscles or movement patterns aimed at improving function and reducing pain. 

Therapeutic Activity:

Patient or functional task eduction. Getting you back to your favorite things as soon as possible.

ONLINE PHYSICAL THERAPY

Duration: 45 minutes

$125

A physical therapy session can be delivered to you in a digital format. With the world being what it is these days, sometimes it is nice to be safe in your home. This is ideal for those with COVID concerns, those that may be immunocompromised, or maybe you find it easier to work into your schedule with an online meeting. Whatever the case may be, this is a convenient service that may be ideal for you.

IN PERSON PERSONAL TRAINING

Duration: 55 minutes $125

45 minutes: $115

For the anyone who wants to focus on wellness and fitness. I can provide professional advice/guidance when returning to the gym or just to keep up your personal fitness/health. Your program will be provided by myself, a Doctor of Physical Therapy who is also a Board Certified Orthopedic Specialist. Getting you to your fitness goals in a safe manner. 

ONLINE PERSONAL TRAINING

Duration: 45 minutes

$100

You can even choose to do your personal training session digitally. Don't have gym equipment? No problem, with body weight or a fully equipped gym, I can find the right exercises to get you fit and moving!

PERSONALIZED 1 MONTH PROGRAM

Cost: $150/month

One 45 minute 1-on-1 digital session per month to review exercises, go over new ones, and answer any questions.

Followed with an independent and personalized exercise program for you to complete for the next month. Which will be progressed/adjusted each month based on your progress and needs.

​

For the cost of one private session, you get benefits for a month of fitness. Specifically: scientific programming for your training and an in-depth and personalized review of the program, to ensure safety and success toward your fitness goals!

Insurance

Do you accept insurance?

Unfortunately, not all physical therapy is created equal. Current insurance reimbursement policies for a physical therapy visit requires physical therapists to treat multiple patients per hour to remain profitable. This can be seen in many healthcare settings: think about how little you see your physician and how long you have to wait to be seen. 

 

This leaves many patients feeling like number, and knowing they are not getting the best individualized care and attention they can.

In my profession, and in my heart, I refuse to continue this cycle. I insist on providing individualized 1 on 1 care. Being available to my patients for questions, concerns, and to guide them through the often frustrating process of getting their health back.

In short, the answer is no. I am an Out-of-Network (OON) provider for all insurances. What this means: I collect payment for the services rendered, as can be seen on the rate sheet above, at the time of service. I then provide a "Superbill" to the patient. This is a document that includes diagnosis and physical therapy treatment codes (CPT codes). The insurance company may then reimburse the patient, based on their OON benefits. You can look up your OON benefits online, or call your insurance provider for more information on your exact reimbursement rates.

Billing Practices

  • Please contact regarding evaluation and treatment information.

  • Sessions (physical therapy or personal training) are billed on a monthly basis. This is determined by the therapist's recommendations, but also what works for your budget and needs. 

​

  • Discounts are provided for those that have multiple sessions per week, and are billed at the beginning of the month, until the subscription is cancelled or you are discharged. 

​

  • Sessions expire after 3 months. 

​

  • Superbill is provided upon completion of sessions for applicable services.

​

  • Medicare patients cannot be provided a Superbill, and can only be seen for health or fitness related goals. Please see FAQ for more information, or contact Jess for further details and questions.
